The movie, like the previous editions, begins on an anticlimax with two men who recognize each other waking up to the realization that they are chained on either side of a table saw. The puppet, enters the scene on a trike telling the two to decide who dies to save the girl they are both dating. The lady on the other end claims that she loves both the men and this leads to a demonstration of anger on both their parts. They both eventually decide to let her take the fall and get ripped into two. The changed detective Mark Hoffman, played by Costas Mandylor, escapes with injuries from the head trap meant to kill him set by the Jigsaw’s ex-wife Jill Tuck, played by Betsy Russell.
Jill realizes that the detective will not give up hunting her and in anticipation, makes her own plans to survive the onslaught. She heads to the police station and engages an officer, Gibson, played by Chad Donella, by telling him he will spill the beans if she is given immunity.
On the other side of town is Bobby Degen, played by Sean Patrick Flannery who survived the Jigsaw, played by Tobin Bell and later wrote a book named S.U.R.V.I.V.E based on his escape experience. After hosting some of the survivors, Bobby is kidnapped and finds himself in a cage and this is where the games begin again among a host of other unrelated scenes.
